Payment

Our fees are based on time, complexity and comparability among the various services. They are designed to reflect the cost of doing business in this community. Every effort is made to ensure fairness and competetiveness with similar services and practices in this community. We are happy to discuss the fees with you at any time.

Insurance plans vary in the amount of total fees they pay for office visits and procedures. Few, if any, insurance plans cover 100% of all charges. Please understand that your insurance reimbursement is a contract between you and your insurer.

Dr. Zondlo is a prefered provider for the following plans:
Medicare, Humana, Blue Cross/Blue Shield,
Mail Handlers, Tricare Standard, Tricare for Life, Tricare Prime, Champus, Integrated Adm. W/C, Choice Managed Network W/C, Industrial Health Management W/C, Medical Review W/C, Provider Management Group W/C, ZRS/Vincam W/C, United Healthcare, Cigna, Aetna, American Heritage, Beech Street Corp., Blue Bell Creameries, CCN Group Health, Evolutions Healthcare, First Health, Heritage Summit, One-Health Plan of Florida, Southcare, Private Healthcare and USA Managed Care. We accept essentially all other health insurance plans as well.

We accept Medicaid only on an individualized basis.

We will assist you with claim filing and all paperwork requiring treatment pre-authorization. Because of the financial difficulties involved with handling accounts recievable, payment is expected at the time of service. We accept personal checks, cash, Visa, Mastercard, and Discover. A 35 % discount is given for cash payment`. If you would like us to bill the insurance carrier, please have your insurance documents available for us to photocopy.

Upon billing your insurance carrier, payment is expected within 30 days. If within 30 days it is not recieved, payment will be due from you at that time. Deductibles and co-payments are due at the time of service. When a procedure is planned we will be happy to approximate the charges for you prior to treatment. Our fees are set independent of insurance companies. Since each insurance company pays differently, we feel it is important that you be informed. We make every effort to ensure our fees are fair and reasonable.

WORKERS COMPENSATION patients must have prior authorization from the employer, the insurance company or the case manager stating that an on-the-job injury was sustained. They then accept financial rsponsibility for your medical expenses. Patients cannot be treated unless the provider agrees to pay for services in accordance with the terms of our contract.
