Office hours are Monday through Friday 8:00 AM-5:00 PM. To allow adequate time for patients to be seen and examined, scheduled appointments are necessary.

If appointments need to be cancelled or rescheduled we ask that you notify us 24 hours in advance. We will also call you 24 hours in advance for verbal confirmation of your scheduled appointment..

We ask that you print out and complete the evaluation forms in the "FORMS" section above to bring to your first clinic visit.

MRI scan reports, CT scan reports, X-Ray reports, and any other pertinent data should also be brought to the first clinic visit.

Without this information, an adequate evaluation of your pain is not possible
